CCISD Athletics Parents and Guardians,
Before athletic, cheer, dance or marching band participation is allowed, students entering 7th, 9th, and 11th grade, as well as, any student who has undergone surgery or been diagnosed with a major illness (cardiac issues, diabetes, epilepsy, etc) in the last year are required to undergo a physical examination. Students who are first-time participants in athletics, cheer, dance, or marching band are also required to undergo a physical examination.
The UIL legislative council has made some changes regarding the pre-participation physical examination for the 2020-2021 school year.
The medical history has new questions specific to electrocardiograms (ECG). If you wish to have your student screened with an ECG, please acknowledge on the form. CCISD will not accept financial responsibility for an ECG. If you check yes, your child will not be allowed to participate in any CCISD practices, competitions, or events until the school’s sports medicine department, coach or sponsor receives a letter of clearance from the doctor that administered the ECG. Forms that are denied will delay student’s participation.
Regardless of their need for a physical examination, students who plan on being involved in athletics, cheer, dance, and/or marching band will be required to complete the medical history questionnaire on the back side of the physical form as well as the auxiliary required paperwork in order to participate during the 2020-2021 school year. Crystal City ISD provides this auxiliary paperwork through the DragonFly Max website,www.dragonflymax.com. DragonFly Max also provides an app for your convenience (the app is blackwith a red hurricane symbol). Medical Questionnaires and physical forms may be obtained on the district website, www.crystalcityisd.org.
When using DragonFly Max, you will need to log on and create an account if you are a first-time user (see attached instruction form). Once you are logged on, click on your child’s profile and you will see a “Prep for 20-21” tab. Dragonfly Max will direct you to the proper paperwork after clicking on the tab.

Students enter the 9th grade with a DragonFly Max account will need to add CCHS to their profile. Click on your profile and look for the“Add Another School” icon in the top right corner. Click on it and search for Crystal City High School or type in DCVQ7N and click GO. An administrator will approve your request.
